Low Vitamin D Levels And Forms Of Neuropathy


Today's post from autoimmunegal.blogspot.com (see link below) refers specifically to Vitamin D deficiency and Sjogren's Syndrome (a systemic autoimmune disease in which immune cells attack and destroy the glands that produce tears and saliva. It is also often linked to rheumatic disorders). Normally the body makes its own vitamin D with exposure to the Sun. The article also talks about the links between low Vitamin D levels and neuropathy. If you have an auto-immune disease/condition, it's possible that you also have low vitamin D levels. That's by no means a certainty but recently research has shown the importance of a stable Vitamin D levels for various medical reasons. People with HIV for instance also often have vitamin D deficiencies, often due to the medications, sometimes leading to, amongst others bone density problems but also peripheral neuropathy. It seems to be one of those subjects that's gaining steam across the internet and it is clear that more research needs to be presented to show how and why vitamin D levels have an effect on auto-immune diseases. In the meantime, if your doctor is willing it will do now harm to check your D levels (along with B12 and others) if you have neuropathic problems.

Those of you who follow me on twitter (
@autoimmunegal) know that I am always tweeting health and medical research studies on autoimmune diseases. Yes, that may make me a little bit of a nerd, but that is OK with me. Along those lines, I recently came across an interesting study (thanks to a tweet from the Sjogren’s Society of Canada) that I thought was very important to share. This study from the Journal of Autoimmunity found a link between primary Sjogren’s Syndrome (pSS) patients with low vitamin-D levels having peripheral neuropathy and having lymphoma.

It is important to mention that this research is the first to find a relationship between low vitamin-D and the risk of lymphoma for those with Sjogren's. There have also been very few studies to date examining vitamin-D levels and their impact on Sjogren's patients. The authors write with regard to neuropathy that “vitamin-D deficiency may be a component in the pathogenesis of neuropathy in pSS.” They then conclude that their research “may warrant the need for a tighter monitoring of vitamin-D among patients with pSS." Low vitamin-D levels have been correlated with autoimmunity in some studies. In addition, vitamin-D has been found to have a protective effect in preventing the development of AI diseases such as lupus, rheumatoid arthritis, autoimmune thyroiditis, and Type 1 diabetes. Some studies have even shown that higher levels of vitamin-D are associated with decreased cancer risk including for non-Hodgkin’s Lymphoma, but as is often the case with scientific research there have been conflicting studies and controversy in this area. Basically vitamin-D seems to be powerful stuff.

When I was diagnosed with Sjogren’s almost two years ago, I was found to have both low vitamin-D levels and severe neuropathy. This study might shed some light on patients like me and serves as big reminder that I best keep taking my vitamin-D supplement. I take a daily over the counter supplement and I do try to have foods high in vitamin-D a few times a week, but that doesn't always raise my vitamin-D level high enough. When my levels are low, my doctor usually prescribes a 12 week course of 50,000 IU of D3 once a week. That usually does the trick to get my vitamin-D in normal range.

The results of this study certainly gives me and others with Sjogren’s MAJOR reasons (lymphoma and neuropathy) to talk to our doctors about vitamin-D and take our supplements if they recommend it. I actually haven’t had my vitamin-D levels (a simple blood test) checked in some time so that will be added to the list to discuss for my next rheumatology appointment in a couple months. BLOOD PRESSURE – Low blood pressure
In this tensed world, tensionless blood (low blood pressure) worries many. Some react to a tense situation with bouts of anger and some others brood and keep on planning to tackle the situation. The people who burst out usually suffer from high BP and the silent sufferer suffers silently with low BP, i.e., thoughtfulness may be the main reason for the slowing down of pressure as far as I have perceived.
Blood vessels which carry blood are elastic in nature – they are free to expand and shrink or collapse readily due to the thin wall and flexible nature according to the flow or pressure of the blood. Contraction of the blood vessel is termed as vasoconstriction and dilatation of the blood vessels is called vasodilatation. Blood flow rate depends on pressure exerted by vessels (resistance), size and length of the blood vessel, flow direction with gravitational force, viscosity of the blood, etc. But the major factors are ability of distention and capacitance of the blood vessels.
Blood pressure is the resistant force faced by the blood against the vessel wall (artery or vein). Arterial pressure has a more significant value than the venous pump (since it is only a backflow). BP usually varies according to the age, sex, heart functions, density of the blood, etc. BP also indicates the strain of the heart – more the pressure, more strained is the heart. Blood pressure alters depending upon the requirement of oxygen for the body. It is mainly controlled by higher functions of the brain in relation to the pituitary, adrenal and thyroid glands. The factors which usually trigger change in blood pressure are mind, water balance, sweat, etc.
Blood pressure is required for blood to reach the brain or other parts of the
body up to the extremities from the base of the heart. It has to travel against the gravitational force to reach the brain. Also, if there is no force, the blood circulation time will be prolonged and it cannot satisfy tissues with its oxygen supply or nutrients. Low pressure also makes the blood vessels more relaxed to accommodate more blood, depriving blood supply to other places further.
Normal blood pressure – The maximum pressure (systolic pressure) is attained at the end of contraction of the heart (systole). The minimum pressure (diastolic pressure) is attained at the end of dilatation or relaxation of the heart (diastole). Pulse pressure is the difference of these pressures. It is raised in elderly persons and diseased persons. Optimum BP (expected to be normal) is sketched as 120/80mmHg. But variation is widely accepted depending upon the age, sex, individual, work, sleep, medicines, etc. The exceeded pressure over 140/90mmHg is considered as hypertension or high BP. The lower pressure, below 100/70mmHg, is considered as low BP which is also known as hypotension.

Low blood pressure – is the pressure reading below 100/70mmHg. Usually sportive or strong persons will have strong heart and stable lower pressure than normal (120/80mmHg) i.e., it would be 110/70 or 100/70mmHg but not below that. This lower pressure usually reduces the risk of cardiac or heart problems. This stable pressure also indicates that heart is well trained to withstand any exertion. Even though low BP rarely creates problem, it needs to be taken count of. Low blood pressure sometimes signals rapid deterioration of health.
Incidence – Mostly short persons are found to have lower BP than taller persons. Also, sedentary obese patients suffer more commonly from low BP than active lean persons. Women have higher incidence of low BP than men. Also, some family members tend to have low BP without any symptoms. It may be normal for them.
Common causes – Researchers have identified lots of causes in correlation with hypotension. They are:
  • Blood – reduced volume, less density, anaemia, blood loss from injury or diseases like cancer and bleeding diathesis
  • Blood vessels – more flexible and relaxed blood vessels, low pumping efficiency of heart, diseases of blood vessels
  • Heart – Hole in heart (patent ductus arteriosis), valve damage or its regurgitation, heart failure, changes in heart rate or rhythm (arrhythmias)
  • Loss of body fluids – due to diarrhoea after food allergy or poisoning, excessive or exhaustive sweating, allergic responses
  • Infections – pancreatitis, peritonitis, pulmonary oedema, septicemia
  • Drugs – medicines used for high BP, diabetes, heart disorders, depression, pain, and sleep can lower the BP. Oral contraceptive pills also tend to lower the BP
  • Hormones – Adrenal insufficiency, pituitary disorders and hypothyroidism
  • Nervous system – stroke, Parkinsonism, autonomic nerves dysfunction
    Surgery – using anaesthesia during surgery can lower the BP
  • Habits – smoking, alcohol and addiction to drugs; coffee, tea, chips and pickles
  • Temperatures – exposure to extreme temperatures of cold and hot (sunstroke)
  • Mind – thoughtfulness, surcharged emotion, brooding
  • Others – deep sleep, being bedridden, breathlessness, saltless food
Symptoms -It is mostly symptomless, since one gets accustomed to it. But a sudden change in pressure due to change of position or other causes may alarm or alert with symptoms of giddiness or instability. The symptoms of low BP are more or less the same as with high BP – hypertension. They are:
  • Fainting or reeling sensation or giddiness
  • Low vitality or weakness
  • Lack of concentration and sleepiness
  • Headache and stiff neck
  • Chest pain and palpitation
  • Shortness of breath or breathlessness
Diagnosis – is important to rule out the underlying cause, if any. Even though BP is easily verified with sphygmomanometer, it is better to investigate blood for Tc, Dc, Esr & Hb and urine for sugar, albumin and deposits and hormones of thyroid, pituatary and adrenals. It is also better to take a chest X-ray and a scan of the abdomen and pelvis.
Complications – When the blood pressure is low, there will be inadequate blood flow causing lack of oxygen to the heart, brain and other organs. This may cause loss of functions. Sudden drop in pressure or further drop in pressure in low pressure patients can make them collapse, thus endangering life. Low diastolic blood pressure increases the risk of dementia in aged persons – Alzheimer’s disease.

Prevention and management – Nutrition and exercise play a major role in regulating blood pressure. So, prevention can easily be attained by the patient by changing one’s lifestyle, losing weight, making dietary changes, exercise and reducing stress. Healthy tips are:
Do’s
  • Check blood pressure regularly
  • Exercise regularly
Take
  • Plenty of juices or water. As the blood pressure falls, heart slows down or vice versa. Water volume can correct it
  • Plenty of vegetables and fruits to energise your body to become active
  • Grains, beans, peas and nuts to boost up blood pressure
  • Non-vegetarian foods like egg, fish, mutton and chicken will also raise the blood pressure. But mind – it is better to have them within limits or otherwise it may increase the cholesterol level and complicate the situation
  • Salt need not be avoided, but it should not be used in excess, since it may hold more water in the tissue spaces
Avoid

  • Smoking, alcohol, coffee and tea
  • Fried items and fast foods
  • Fasting
  • Hot sun and extreme cold weather
  • Vigorous exercises which causes profuse sweating
  • Unnecessary drugs
  • Being overweight
Care while changing Posture – To avoid giddiness or falling and to gain stability in no time, rise slowly from the bed or chair and move about slowly after gaining stability without any tension or fear. Develop these postural changes as habits, so as to avoid a fall.
As a general rule –
  • Eat well, work well and sleep well
  • Be active and be busy
  • Don’t be thoughtful and accumulate stress
Finally, it is well said that spend more money to increase volume of blood than to contaminate it with drugs, i.e. eat and drink well.
General treatment – High BP patients are usually managed easily with medicines for life time, but no one prescribes or provides treatment for low blood pressure, unless otherwise it shows symptoms like dizziness, weakness, fatigue or palpitations, since medicines used for increasing BP will create a strain on the heart and cause side-effects.
Hypotension is an ongoing process of chronic fatigue. The transient drop or doctors advice oral rehydration therapy or IV (intravenous) drips to increase blood volume. In case of reduced pulse or heart rate, treatment is concentrated in boosting the heart functions by increasing its rate and efficiency.

HOMOEOPATHIC REMEDIES FOR LOW SPERM COUNT


Low sperm count means that the fluid (semen) you ejaculate during an orgasm contains fewer sperm than normal.
A low sperm count is also called oligospermia . A complete absence of sperm is called azoospermia. Your sperm count is considered lower than normal if you have fewer than 15 million sperm per milliliter of semen.
Having a low sperm count decreases the odds that one of your sperm will fertilize your partner's egg, resulting in pregnancy. Nonetheless, many men who have a low sperm count are still able to father a child.
Causes-- The production of sperm is a complex process and requires normal functioning of the testicles (testes) as well as the hypothalamus and pituitary glands — organs in your brain that produce hormones that trigger sperm production. Once sperm are produced in the testicles, delicate tubes transport them until they mix with semen and are ejaculated out of the penis. Problems with any of these systems can affect sperm production.
Also, there can be problems of abnormal sperm shape (morphology), movement (motility) or function.
However, often the cause of low sperm count isn't identified.
Medical causes
Low sperm count can be caused by a number of health issues and medical treatments. Some of these include:
·         Varicocele. A varicocele (VAR-ih-koe-seel) is a swelling of the veins that drain the testicle. It's the most common reversible cause of male infertility. Although the exact reason that varicoceles cause infertility is unknown, it might be related to abnormal testicular temperature regulation. Varicoceles result in reduced quality of the sperm.
·         Infection. Some infections can interfere with sperm production or sperm health or can cause scarring that blocks the passage of sperm. These include inflammation of the epididymis (epididymitis) or testicles (orchitis) and some sexually transmitted infections, including gonorrhea or HIV. Although some infections can result in permanent testicular damage, most often sperm can still be retrieved.
·         Ejaculation problems. Retrograde ejaculation occurs when semen enters the bladder during orgasm instead of emerging out of the tip of the penis. Various health conditions can cause retrograde or lack of ejaculation, including diabetes, spinal injuries, and surgery of the bladder, prostate or urethra.
·          
  • Certain medications also might result in ejaculatory problems, such as blood pressure medications known as alpha blockers. Some ejaculatory problems can be reversed, while others are permanent. In most cases of permanent ejaculation problems, sperm can still be retrieved directly from the testicles.
  • Antibodies that attack sperm. Anti-sperm antibodies are immune system cells that mistakenly identify sperm as harmful invaders and attempt to destroy them.
  • Tumors. Cancers and nonmalignant tumors can affect the male reproductive organs directly, through the glands that release hormones related to reproduction, such as the pituitary gland, or through unknown causes. Surgery, radiation or chemotherapy to treat tumors can also affect male fertility.
  • Undescended testicles. During fetal development one or both testicles sometimes fail to descend from the abdomen into the sac that normally contains the testicles (scrotum). Decreased fertility is more likely in men with this condition.
  • Hormone imbalances. The hypothalamus, pituitary and testicles produce hormones that are necessary to create sperm. Alterations in these hormones, as well as from other systems such as the thyroid and adrenal gland, may impair sperm production.
·         Defects of tubules that transport sperm. Many different tubes carry sperm. They can be blocked due to various causes, including inadvertent injury from surgery, prior infections, trauma or abnormal development, such as with cystic fibrosis or similar inherited conditions.
Blockage can occur at any level, including within the testicle, in the tubes that drain the testicle, in the epididymis, in the vas deferens, near the ejaculatory ducts or in the urethra.
·         Chromosome defects. Inherited disorders such as Klinefelter's syndrome — in which a male is born with two X chromosomes and one Y chromosome instead of one X and one Y — cause abnormal development of the male reproductive organs. Other genetic syndromes associated with infertility include cystic fibrosis, Kallmann's syndrome and Kartagener's syndrome.
·         Celiac disease. A digestive disorder caused by sensitivity to gluten, celiac disease can cause male infertility. Fertility may improve after adopting a gluten-free diet.
·         Certain medications. Testosterone replacement therapy, long-term anabolic steroid use, cancer medications (chemotherapy), certain antifungal and antibiotic medications, some ulcer medications and other medications can impair sperm production and decrease male fertility.
·         Prior surgeries. Certain surgeries might prevent you from having sperm in your ejaculate, including vasectomy, inguinal hernia repairs, scrotal or testicular surgeries, prostate surgeries, and large abdominal surgeries performed for testicular and rectal cancers, among others. In most cases, surgery can be performed to either reverse these blockages or to retrieve sperm directly from the epididymis and testicles.
Environmental causes
Sperm production or function can be affected by overexposure to certain environmental elements, including:
·         Industrial chemicals. Extended exposure to benzenes, toluene, xylene, herbicides, pesticides, organic solvents, painting materials and lead might contribute to low sperm counts.
·         Heavy metal exposure. Exposure to lead or other heavy metals also can cause infertility.
·         Radiation or X-rays. Exposure to radiation can reduce sperm production. It can take several years for sperm production to return to normal. With high doses of radiation, sperm production can be permanently reduced.
·         Overheating the testicles. Elevated temperatures impair sperm production and function. Although studies are limited and are inconclusive, frequent use of saunas or hot tubs might temporarily impair sperm count.
Sitting for long periods, wearing tight clothing or working on a laptop computer for long stretches of time also might increase the temperature in your scrotum and slightly reduce sperm production.
Health, lifestyle and other causes
Other causes of low sperm count include:
·         Drug use. Anabolic steroids taken to stimulate muscle strength and growth can cause the testicles to shrink and sperm production to decrease. Use of cocaine or marijuana might reduce the number and quality of your sperm as well.
·         Alcohol use. Drinking alcohol can lower testosterone levels and cause decreased sperm production.
·         Occupation. Certain occupations might be linked with a risk of infertility, including welding or those associated with prolonged sitting, such as truck driving. However, the data to support these associations is inconsistent.
·         Tobacco smoking. Men who smoke might have a lower sperm count than do those who don't smoke.
·         Emotional stress. Severe or prolonged emotional stress, including stress about fertility, might interfere with hormones needed to produce sperm.
·         Weight. Obesity can impair fertility in several ways, including directly impacting sperm and by causing hormone changes that reduce male fertility.
·         Sperm testing issues. Lower than normal sperm counts can result from testing a sperm sample that was taken too soon after your last ejaculation; was taken too soon after an illness or stressful event; or didn't contain all of the semen you ejaculated because some was spilled during collection. For this reason, results are generally based on several samples taken over a period of time.

HOMOEOPATHIC REMEDIES
ARGENTUM NITRICUM 6—Argentum nitricum is an effective remedy for low sperm count  with erectile dysfunction. Erection fails when coition is attempted . Coition is painful and desire wanting. Argentum niticum patients are melancholic, apprehensive of serious disease . They are impulsive , wants to do things in a hurry. The patient feels time passes too slowly. The patient complaint of flatulence. In Argentum nitricum there is a great craving for sweets.
 DAMIANA Q- Damiana is an excellent medicine for increasing the sperm count.   Sperms are absent in semen. Damiana acts magically and increase the sperm count. It also corrects erectile dysfunction.
CONIUM MACULATUM 200—Conium maculatum is the top medicine for low sperm count with orchitis.Conium is selected when there is swelling and enlargement of the testes. A cutting type of pain is felt in the testes. The testes are also indurated and hardened.

STAPHYSAGRIA 30- Staphysagria is another suitable remedy for low sperm count  from orchitis. It is indicated when burning, shooting, tearing or drawing pain is present in the inflamed testes. Staphysagria is also indicated for orchitis following mumps. Persons needing Staphysagria also show constant dwelling on sexual subjects.

RHODODENDRON 30-Rhododendron is effective for low sperm count with orchitis.Rhododendron is helpful when there is swelling and severe pain in the testes that worsens from slight touch. Contusive, crushed and drawing pain is felt in the testes along with swelling.Rhododendron is also useful useful for low sperm count  resulting from hydrocele.The testes are swollen , painful and drawn up.

AURUM MET. 30-Aurum Met is useful for low sperm count with hydrocele. Along with hydrocele, high sex drive and nocturnal pollutions are also present.

IODUM 30-Iodum is a wonderful remedy when aching, pressing and forceful pain is present with hydrocele.

AGNUS CASTUS 30-Agnus Castus is a prominent remedy for low sperm count with decreased sexual powers. There is inability to attain erections and the genitals are relaxed, shrivelled, cold and flaccid. An aversion to sex is also there.

CALADIUM 3X-- Caladium is also useful  for low sperm count  with weak sexual powers. The person lacks physical power to have sex although the desire is present. Mental depression may accompany the condition. Caladium is also considered for diminished sexual power from tobacco abuse.

COBALTUM 30- Cobaltum is effective for low sperm count  with erectile dysfunction. Emissions without erection. Backache in lumbar region and weak legs. Brown spots in genitals and abdomen.

SELENIUM 30-Selenium is very effective for low sperm count with diminished sexual power. There is insufficient, weak and slow erections.  Selenium is also helpful in a person with low sperm count who has involuntary seminal emissions at night, during urination or stools. Semen is thin , cold and odourless.On attempting coition , penis relaxes. Increased  desire with decreased ability.

Cannabis With Low THC Component Effective For Nerve Pain


Today's post from hcplive.com (see link below) discusses an interesting cannabis study directed at people living with significant neuropathic pain. The conclusions are: that compared with the placebo and after taking into account the psychoactive effects of vaporised cannabis, trial participants experienced a marked reduction in pain and that from a relatively low THC dosage. That's nothing new you might think; we've accepted that cannabis is an effective analgesic a long time ago but it is rare to see trials specifically for neuropathy patients and for that reason, it's worth publishing on this blog. The more concrete evidence, the better!


Medical Marijuana: Low THC Sufficient for Neuropathic Pain

Study results on the analgesic effect of cannabis have been mixed over the years. However, many states allow prescriptions for medical marijuana for a variety of health conditions, including pain. A recent study published in The Journal of Pain looked at the effect of marijuana treatment on neuropathic pain.

A team of researchers based in California conducted eight-hour human analyses to determine pain level outcomes with the use of vaporized cannabis. The cohort included 42 patients with neuropathic pain related to an injury or disease of the spinal cord. Although most of the participants were also on a traditional treatment, they were still experiencing pain.

The patients were randomized to take four puffs of vaporized cannabis that contained either placebo, 2.9%, or 6.7% delta 9-tetrahydrocannabinol (THC). Three hours later, the participants took another four to eight puffs – the number of puffs was decided by the patient in order to reduce the placebo effect.
  

An 11-point numerical pain intensity rating scale showed that the vaporized cannabis produced a significant analgesic response.

“When subjective and psychoactive side effects (eg, good drug effect, feeling high, etc) were added as covariates to the model, the reduction in pain intensity remained significant above and beyond any effect of these measures,” the report said.

Psychoactive side effects did occur as a result of the delta 9-THC, which is expected as marijuana is a brain-altering drug. The researchers said that neuropsychological performance was challenging to measure because many of the participants had disabilities.

Nevertheless, two of the highlighted findings included:


There was significantly more pain relief with active cannabis than placebo.
The two active doses did not have significantly different outcomes when it came to analgesic potency.

Since the higher active dose didn’t appear to provide more pain relief, the authors advise that patients with neuropathic pain associated with spinal cord injury or disease should be provided with the lower dose in order to avoid potential risks.
